Carrie Daly, APN-CNP, an advanced practice clinician specializing in hospital medicine at Endeavor Health, has more than four decades of nursing experience and over 30 years as a nurse practitioner. She founded and leads a successful subacute rehabilitation program that helps patients recover and transition safely from hospital to home. Carrie believes in caring for the whole person, mind, body and spirit, and takes pride in mentoring others to deliver compassionate, patient-centered care.
