The community programs at Endeavor Health are designed to uplift every member of our communities. Through these programs, we provide low-cost, accessible care options – from routine care and specialty services, to a variety of wellbeing programs and support groups – all located within Chicagoland or virtually.

Community Health Center
Provides free and low-cost care for individuals with limited access to private healthcare. Services include internal medicine, OB/GYN, general surgery, orthopaedics and diabetes education.
Available to: Residents of Evanston and surrounding areas who meet predefined geographic and economic guidelines

Community Resource Center (CRC)
Supports under-resourced individuals and immigrant families in the northwest suburbs with multilingual services, referrals and other services in partnership with Partners for Our Communities (POC).

Community Health Worker (CHW) at Northwest Community Hospital
Meets bedside with low-income, primarily Spanish-speaking patients at Northwest Community Hospital to help them understand and comply with discharge instructions, find medical homes and receive other resources.
Community Nursing
Nurses from Northwest Community Hospital provide free chronic disease education, blood pressure screenings and diabetes support at three community locations.

DuPage Health Coalition
Provides assistance to individuals and families with low income in DuPage County seeking low-cost care, medications and health insurance.
Available to: DuPage County residents with lower income or no insurance

We offer three programs at DuPage Health Coalition:
- Access DuPage

Evanston Township High School Health Center
School-based health center at Evanston Township High School (ETHS) that provides free care for all students.

Family Care Center
Comprehensive care for the whole family, from annual wellness visits, physicals and illness visits to chronic condition management and behavioral health support. Available on a walk-in basis or via appointment. Please call for walk-in availability.

Financial assistance programs/applications
Endeavor Health financial counselors work with patients to create manageable payment plans and connect them with financial assistance programs that may help lower the cost of care.
Healthcare Transformation Program
Expands access to care by integrating specialty services and support into local Federally Qualified Health Centers (FQHCs), helping reduce disparities and barriers for underserved communities.
Available to: Patients of our referring FQHCs, or patients of Swedish Hospital with medicaid, no insurance, or private insurance with the inability to pay for out of pocket costs

Provides low-cost oral healthcare for community members with low-income.
Available to: Residents of Elk Grove, Palatine, Schaumburg and Wheeling Townships who financially qualify in their residing township

A free, 24/7 crisis helpline for any community member seeking mental health crisis support reachable at (847) 432-5464.
Bridges early childhood and adolescent program
Offers comprehensive mental healthcare to insured and uninsured children ages 3-18.
Available to: Children residing in the northern suburbs or Chicago north side

The Care Center at Edward Hospital provides medical care in a calm and caring environment for children and adolescence.

Community Behavioral Healthcare
Provides individual and group therapy for underserved populations, reduces stigma around mental health and builds provider capacity to support our diverse community.

Offers comprehensive treatment for individuals facing substance use disorders and co-occurring mental health disorders.

Haymarket Center
Haymarket Center works within crisis teams at Edward and Elmhurst Hospitals to talk with patients about substance use issues and help connect them with the appropriate care.
Available to: Patients of Edward Hospital or Elmhurst Hospital
Outpatient Clinic for Adult Community Services
Provides a wide range of psychiatric and case management services, from assessments and treatment planning to assistance with reduced free prescription programs.
Available to: Adults (18+) dealing with chronic and severe mental illness who require medication management

Perinatal wellness program
Offers a 24/7 free crisis hotline (MOMS Line) at (866) 364-6667. This provides support through Parentline, short-term virtual psychotherapy, a screening, mental health triage and referral program.
Available to: The MOMS Line and Parentline are available to birthing persons and their family members across Illinois, regardless of where they receive their care. The perinatal screening, triage and referral program is available to birthing persons who receive their care at NorthShore and Swedish locations, as well as select affiliate practices.

Suboxone clinic
Provides weekly treatment for opioid addiction, helping individuals regain control of their lives and improve their health.
Location: Emergency Department at Swedish Hospital

Smoking cessation program
Provides an 8-week, comprehensive support group combined with personalized strategies to help participants quit smoking through self-awareness and guided behavioral change.
Available to: All community members
Location: Hybrid. Sessions are held virtually and in person. In person sessions take place at Swedish Hospital.

Tobacco cessation
Free tobacco cessation counseling and support to aid with lung cancer prevention provided at Northwest Community Hospital.

Young Hearts for Life
Young Hearts for Life (YH4L) is the largest cardiac detection program in the United States for preventing sudden cardiac death for youth. At Endeavor Health, we’re honored to support YH4L with financial contributions and volunteer support.
Available to: Available to students enrolled at participating schools
Location: Primarily in DuPage County. Expanding into Will, Kane, Cook and Lake County.
Community Outreach Registered Dietitians (CORD)
Free nutrition and wellness classes provided by our registered dietitians to the community. Classes include the EAT Series, Community Flavors, Living with Diabetes and the Diabetes Prevention Program.
