Here’s a checklist to help you prepare:
- Photo ID: Bring a valid ID to verify your identity and ensure accurate medical records and billing.
- Insurance information: Verify insurance details and bring your card with you.
- Medical history: Bring any relevant documents or records of past treatments, diagnoses or surgeries.
- Medication list: Include all prescription and over-the-counter medications, supplements and dosages.
- List of questions: Write down any questions or concerns you have about your health, treatment options or wellness goals.

- Call your provider’s office directly to make, change or cancel an appointment.
- Log in to your patient portal to manage your appointments online.

We have a few different patient portals, depending on where you receive care. Visit our patient portal page to find the right portal and get help with login issues, technical support or managing your account.

If you have a medical question or concern, please reach out to your doctor or healthcare provider directly. You can contact their office by phone. If your provider is part of Endeavor Health Medical Group, you can also send a message through your patient portal.

We provide an online Primary Source Verification service for hospitals, healthcare organizations and credentialing agents. You can generate verification letters for our medical staff and advanced practice providers.
We offer a variety of communication support services — including in-person, phone, and video interpretation in multiple languages (including ASL), as well as TTY devices for people who are deaf, hard of hearing or have speech impairments. These services are available at no cost to patients or caregivers.

Birth certificates are issued by the county or the Illinois Department of Public Health (IDPH), not by our organization.
- If your child was recently born at one of our hospitals, you should have received a form with instructions before discharge on how to request an official birth certificate from the county.
- If you need a copy of a birth certificate that has already been issued in Illinois, visit the IDPH website and follow the directions to make your request.
