The team serves as a compassionate liaison to assist with investigation, internal review, and resolution, ensuring your voice is heard.

We’re here to support your feedback, but please note that we’re not able to provide medical advice. For questions about symptoms, medications or follow-up care, please contact your provider directly. If your feedback relates to billing, scheduling, medical records or other services, we’ll direct you to the appropriate department through our contact us page.

The Patient Experience and Advocacy team is available Monday through Friday by phone, email or mail. Please provide the following information:

  • A brief description of the experience

*If you are not the patient, please be aware that we will need to obtain the patient’s consent before discussing any matters related to their care.
