We take great care to keep your medical records confidential and secure. There are several ways to request your records. Choose the location where you received care to find the appropriate steps and contact information.
Request methods by location
If you received care at Evanston Hospital, Glenbrook Hospital, Highland Park Hospital, Skokie Hospital or Swedish Hospital — or at an Endeavor Health doctor’s office or immediate care location in the northern suburbs (including Evanston, Northbrook, Highland Park, Skokie) or on Chicago’s north side (including Lincoln Square, Lakeview, Uptown, Jefferson Park) or nearby areas.

Patients and providers can request medical records using our online tool. Records can be sent electronically or by mail.

NorthShoreConnect
If you have an active NorthShoreConnect account, you can request your records through the portal.
- Log in and go to Your Menu > search Medical Record Request
- Records are typically available within 1–3 business days
- Requested records will be available in Document Center > Requested Records
Note:
- Records can only be released to the patient.
- Radiology images can be emailed separately.
- Cardiac imaging cannot be sent via email or to NorthShoreConnect. See contact info below for assistance.

Request in person
- You may request and receive copies of your records at one of our hospital locations. Please bring:
- A completed Authorization for Disclosure of Confidential Information form (or fill one out onsite)
- A government-issued photo ID
Note:
- If someone is picking up your records for you, the form must be completed and signed by the patient, with the recipient’s name listed in the “I authorize NorthShore University HealthSystem to release to” section.

- Use for: Requesting changes to your medical record
- How to submit: Fill out the Amendment Request Form and follow the directions in the document on how to submit.
Notes:
- Endeavor Health will review the request, but clinical notes are part of your physician(s) and care team’s assessment and therefore any amendment requests will be at Endeavor Health discretion based on a review of the note.

Important:
- As of May 16, 2025, Elmhurst Hospital no longer accepts mail/in-person requests. Please use the online request option for Elmhurst Hospital.
- Off-site locations cannot burn radiology CDs — this service is only available at Edward Hospital.
What to bring:
- A completed Authorization to Use and Disclose Health Information form (or fill it out during your visit).
- If picking up a radiology CD for someone else, the form must be signed by the patient and include your name in Section 4.
- A valid photo ID (e.g., driver’s license).
Frequently asked questions
- For access to past visit information, medication records and older lab results, visit Diamond Headache’s former patient portal. Please do not use the old portal to message your providers or to request medication refills.
- To request medical records from Diamond Headache Clinic for care provided before Oct. 31, 2025, download the request form.
To obtain a copy of a birth or death certificate, contact the Illinois Department of Public Health.
To check the status of a Social Security card for your baby, contact your local Social Security office.
- The patient or the patient’s legally authorized representative (legal documentation is required)
- The parent of a patient under the age of 18
- The parent and teenager (ages 12-17) for records that contain mental health or other protected diagnoses contained in the health information
- A legally-appointed executor or administrator of an estate if the patient is deceased (legal documentation, court order or small estate affidavit, and a death certificate are required)
You have the right to access, review and get copies of your medical record. Except where permitted by law, your medical record cannot be released without your consent.
