Endeavor Health has launched an online survey to gauge the health needs of residents in the hospitals’ surrounding communities. This initiative is a critical component of the health system’s Community Health Needs Assessment (CHNA), which is conducted at least every three years to identify key health challenges, service gaps and other factors that influence the overall health and well-being of community members.

By engaging directly with the community through this survey, Endeavor Health intends to gather essential insights that will inform its strategic planning, resource allocation, and the development of priority health improvement programs and partnerships.

“We are dedicated to understanding the unique health needs of our community directly from the source—the people we serve,” said Kate Lawler, System Vice President for Community Impact and Engagement. “This survey is more than a tool; it’s a bridge to deeper understanding and providing a more safe, seamless and personal experience.”

Residents of Endeavor Health’s service area are encouraged to participate in this confidential survey, which will take about 10 minutes to complete. The questionnaire covers a broad range of health topics, including access to medical care, chronic diseases, mental health, social determinants of health and more. It also seeks participants’ views on the top health priorities for the community. Responses will be kept anonymous and will play a crucial role in guiding Endeavor Health’s future health initiatives and outreach efforts.
