Access patient information with EpicCare Link

EpicCare Link is a web-based portal providing approved partners — including affiliate offices, skilled nursing facilities (SNFs), home health agencies, Endeavor Health preferred partners, research partners, billing companies, etc. — with real-time access to patient charts at any of the hospitals in the Endeavor Health system. Getting access: The five step process

Access is granted first to your organization (site) and the site administrator. The rest of your site’s end users will be requested by the site administrator.

Each site must designate at least one site administrator (two are recommended).

Once approved, site administrators can:

  • Submit access requests for new end users
    • Access should be requested only for individuals that have a need to use the system and will keep their accounts active
  • Terminate end user access when no longer needed
    • Access should be removed promptly when an individual no longer requires EpicCare Link
  • Reset passwords for end users at their assigned site
  • Serve as first point of contact for access questions
  • Submit quarterly site verifications to confirm active users and providers

Have these details and documents ready before you begin to ensure a seamless application process. To locate the appropriate contract and confidentiality agreement, please choose the description that best matches your entity from the approved partner descriptions section found below.

For the site application:

  • Legal entity name and all locations (address, phone number, fax number, etc.)
  • Completed EpicCare Link agreement (signed by an authorized approver)
  • Site administrator details: Name, email address, and job title
  • External partner agencies will need NPI number and a direct mailbox from an accredited HISP.

For individual user requests:

  • Signed confidentiality agreement for each user
  • Required personal details: Full name, home address, phone number, and work email address

 

Navigate to the site access request. You will first be prompted to enter details about the entity requesting access. You will also be required to upload the signed contract to the electronic request form. The contract must be signed by a medical director, administrator, or an individual with authority to approve access for your entire organization.

The next section of the application will be where the site administrator(s) and user requests are entered. At this point of the application process, please only enter your site administrator(s) requests. Once approved, the site administrator may submit requests for individual end users.

  • When requesting new users, you will be prompted to select the type of user.
  • A signed confidentiality agreement must be uploaded to the electronic request form for all user access requests.
  • Be sure to check the box on the electronic request form to indicate if the user should be designated as a site administrator. You will not be able to submit your application unless you have designated at least one individual as your site administrator.

On the verification screen, you will be required to agree to the  terms & conditions and policy statement, complete the captcha, type your name as the requestor, and select the primary site administrator that should receive communications about the request. 

Once the request is submitted, the designated site administrator will receive a verification email from no-reply [at] endeavorhealth.org (no-reply[at]endeavorhealth[dot]org) with a subject of Verify Email Address For Endeavor EpicCare Link. Please ensure this email address is added to your allowed senders list so it doesn’t go to your spam folder. Upon clicking the verification link, the site administrator will receive another email with a subject of Confirmation of new account request for Endeavor EpicCare Link. This email will contain a reference code for the request that should be retained as it will be needed later for setting the password upon first login to the EpicCare Link, if the access is approved.

Once the new site request has been approved by Endeavor Health, the site administrator will receive an email with a subject of Complete Endeavor EpicCare Link account setup and any other end users that were requested will receive an email with the subject New Endeavor EpicCare Link account provisioned and will contain their user ID and inform them they should contact their site administrator to complete the setup. The site administrator should click the link found in their email. On the account setup screen, the site administrator should enter the user ID that was provided as well as the reference code that was provided in the confirmation email and click Accept. Enter your new password twice and click Accept. Your password must contain at least 8 characters, at least one number, at least one uppercase, at least one lowercase, and it cannot contain your account or full name. You will receive an email with the subject Endeavor EpicCare Link Password Change Notification to confirm the change.

Once the new password is set, you will be directed to the EpicCare Link login page where you can now login with your user ID and the new password you have set. You can now access site administrator actions by clicking on admin. When you access the admin menu, you will be prompted to set the initial password for the other end user(s). After creating a temporary password for an end user, the site administrator will receive a popup message with all the necessary login credentials that need to be communicated to the end user. Upon the end user’s first login, they will be prompted to change their password.

This is a security requirement for identity verification within the Epic system to ensure that each account is unique and HIPAA-compliant.

Once the site access request and contract are submitted, review typically takes 10-14 business days. Turnaround time could be longer depending on the specific request and approval required. User accounts are usually activated within 5 business days of the approved site administrator’s request

To maintain security, Site administrators must perform a quarterly verification of all active users. Access should be removed immediately when a staff member no longer requires the system or leaves your organization. Failure to complete the quarterly site verification will result in your end users’ access being temporarily blocked until the verification is completed.

No. EpicCare Link is web-based. You only need a PC or Mac and a current browser (Chrome, Firefox, Edge or Safari). Note: For security, mobile devices and Internet Explorer are not supported.

No, this is a standalone portal. However, clinical data can be printed or copied into your local records to support care coordination.
