On November 7, 2026, Edward-Elmhurst MyChart will be replaced with MyEndeavor, our new patient portal. While its core features will remain the same, we have outlined several important updates below to help you prepare for this change.

MyEndeavor access & settings
You can begin using MyEndeavor on Saturday, November 7, 2026. Your Edward-Elmhurst MyChart account and recent medical history will automatically transfer to MyEndeavor.
On November 7, Edward-Elmhurst MyChart will permanently switch to a limited access mode, whether you access it using a web browser or mobile app. This change to limited access mode means you may use Edward-Elmhurst MyChart to view certain medical information, such as health records and previous messages, and pay bills for service prior to November. 7, but all other features will be transitioned and require you to use our new patient portal, MyEndeavor.
Important note: Starting Thursday, November 5, you will lose the ability to send messages or order prescription refills on Edward-Elmhurst MyChart. To resume messaging your care team or requesting refills online, you must log in to your MyEndeavor account beginning November 7. If you need assistance during the transition period on Nov. 5 and 6, please call your doctor’s office directly.
Starting November 7, you can access your account from your computer or mobile device:
- Online: Access the portal through your web browser.
- Mobile app: Log in directly through the new MyEndeavor app.
Forgot your login information?
If you cannot remember your username or password, you can recover your account online.
Most users can sign in to MyEndeavor using their current Edward-Elmhurst MyChart username and password.
Because of our transition to MyEndeavor, some patients’ usernames must be changed. This will happen in two scenarios:
- Your username is already taken: To ensure every account in our new patient portal is unique, you may be assigned a new username. Your password will remain the same.
- Your username doesn't meet security standards: If your current username is too short or contains unsupported special characters, you will need to create a new, secure username.
If your username is affected, we will email you specific instructions before November 7. Once the transition is complete, if you forget your username or need help signing in, you can easily request a username reminder on the MyEndeavor sign-in page, or you can contact MyEndeavor Support for further assistance.
If you use multiple Endeavor Health patient portals
If you currently have active accounts for more than one Endeavor Health patient portal, your accounts will automatically merge. You will only need to use one username and password going forward.
Please use the following rules to determine which login information to use for MyEndeavor:
- If you have an active NorthShoreConnect account: You will use your NorthShoreConnect username and password to log in. If you have other accounts, your other account information will automatically transfer over on November 7.
- If you do NOT have NorthShoreConnect but have an Edward-Elmhurst MyChart account: You will use your Edward-Elmhurst MyChart username and password. If you also have an NCH MyChart account, your NCH information will automatically transfer over.
- If you ONLY have an active NCH MyChart account: You will use your NCH MyChart username and password.
Unsure which login to use?
If you cannot remember your username or password, you can recover your account online. You can contact MyEndeavor Support for further assistance.
After November 7, you will need to download the new MyEndeavor app on your mobile device from the Apple App Store or Google Play to manage your care on the go.
The Edward-Elmhurst MyChart app will become limited access with very limited features. You may decide to delete the Edward-Elmhurst MyChart app from your device.
The way you receive appointment reminders and health notifications is changing. You will still receive these important updates, but they will now come from a new email address and new phone numbers.
- Emails: Portal notifications will now come from info [at] my.endeavorhealth.org (info[at]my[dot]endeavorhealth[dot]org). Please add info [at] my.endeavorhealth.org (info[at]my[dot]endeavorhealth[dot]org) to your email contacts or "safe senders" list to ensure these important health updates don't end up in your spam folder.
- Text messages: Updates about your health and care visits, like appointment reminders, tests results, and other health information, will come from 91356, medical billing updates from 25064 and wellness updates from 61863. Please note that if you text “STOP” to any of these, you will no longer receive these messages.
Your communication preferences
Your general email and text messaging opt-in status will automatically transfer to MyEndeavor. However, your specific topic preferences (how and when you receive individual alerts like appointment confirmations or billing statements) will reset to our new standard defaults.
To review and update your settings:
- Log in to MyEndeavor.
- Open the main menu, scroll to the Settings section, and select Communication Preferences.
- Review each notification category and use the toggles to turn your preferred delivery methods (text message, email or phone call) on or off.
- Important: Scroll to the very bottom of the screen and click Save changes. If you do save your preferences and will not receive important notifications.
Improved features & settings
Your appointment scheduling options are expanding. You can now conveniently search, schedule and manage appointments with more providers and services across all Endeavor Health locations.
Additionally, you can schedule Immediate Care in-person visits online. This allows you to choose a specific appointment time and location before you leave your home, minimizing your wait time.
We are making it easier to connect with your care team. You can now send messages to a wider range of clinicians you have previously met with, including dietitians, midwives, social workers and more.
When you send a message, you will also notice the subject options have been updated and simplified. This makes it easier to select the right topic and ensures your question reaches the correct care team member faster.
Updates to how medical messages are billed
Most responses from your care team remain available at no cost. However, if a response to a medical question requires clinical expertise or complex decision-making, your doctor may provide medical advice and your insurance may be billed. Before sending a non-urgent medical question, you will be asked to acknowledge that you may be charged.
- Messages that may be billed: Include reporting a new symptom, asking for a medication adjustment, reporting a symptom flare-up or asking a question that requires extensive chart review.
- Messages that are generally not billed: Include routine medication refill requests, simple updates on your recovery or questions that can be easily answered by clinical support staff.
When a child turns 12, Friends and Family Access (also known as proxy access) will automatically transition to limited access. Parents and legal guardians can continue supporting their child, but certain medical information will be hidden to protect the teen's privacy unless full access is explicitly granted.
Additionally, patients ages 12 to 17 can now request their own MyEndeavor account. This allows them to take a more active role in their healthcare and communicate privately with their care team.

If you plan to deliver your baby at an Endeavor Health hospital, you can securely pre-register for your admission directly through your MyEndeavor account. Completing these steps in advance will save you time when you arrive to deliver.
Pre-registering in MyEndeavor:
- Share your details: Provide your estimated due date, select your delivery location and update your personal and insurance information.
- Complete forms in advance: Review and sign required admission forms securely from home.
To pre-register for your delivery:
- Log in to MyEndeavor.
- Open the main menu, scroll to the Find Care section, and select Register for My Delivery.
Having a photo in your record helps us ensure we are providing the right care to the right person. You may be asked to have your profile photo taken during registration or check-in. Having a patient profile photo is optional. If you prefer, you can securely upload your own photo directly in MyEndeavor.

Updates to your records & billing
To give you a clearer picture of your health, most test results, diagnoses and clinical notes will be shared with you in MyEndeavor by default.
Your test results are generally released to your patient portal as soon as they are available. If you prefer to review your results with your doctor before seeing them online, you can adjust your MyEndeavor settings to delay all your test results for four days.
Note: As permitted by Illinois law, certain highly sensitive results are always held until your doctor can discuss them with you directly.

Migrating your health history
To ensure you have continuous access to your care journey, most of your historical health records will securely transfer to MyEndeavor.
How your history transfers depends on the type of information:
- Essential health info (all records): Your documented allergies, past immunizations, active medical issues, lifetime labs, such as genetic tests, and medication lists will transfer completely with no time limit.
- Specialty screenings (past 10 years): Your past colonoscopies, sleep studies and other testing will transfer going back 10 years.
- Most everyday records (past 5 years): Your routine lab tests, imaging results (X-rays, CT scans and MRIs), doctor's visit notes, historical letters, scanned documents, anesthesia summaries, implant records and pregnancy history will transfer going back 5 years.
Accessing older records
For a limited time, your previous patient portal will remain available in a limited access mode. While your most important health history will securely transfer to MyEndeavor, you can still log into your previous portal to view older records if needed.
Note: our HIM Department will have access to all of your medical records from Endeavor Health and can assist you with identifying any specific records you are looking to access. Where you pay your bill depends on the date of your visit:
- Services received on or before Nov. 6: Continue making payments through Edward-Elmhurst MyChart. Your account will remain in read-only status, but you will still be able to pay bills for past care there.
- Services received on or after Nov. 7: Log in to MyEndeavor to view and pay your bills.
Our billing is changing to give you a more transparent experience. Previously, some families received combined household statements. Going forward, each adult patient will have their own billing account and statement.
This means you may receive separate statements where you currently receive one:
- Patients aged 18 and older will receive their own individual billing statement.
- If you are the designated guarantor for one or more children under age 18, you will receive one billing statement that includes both your care and the care of your minor children.
If you need to manage or pay bills for a spouse, adult child, dependent or other adult, they can invite you to access their billing information by setting up an authorized billing user.

Your billing statement may separate charges into two categories based on the type of care you received. You may see both categories on your statement for the same visit because they cover different parts of your care:
- Hospital accounts: Charges related to facility services, such as room charges, nursing care, medications and medical supplies.
- Professional accounts: Charges from doctors or other healthcare professionals for their services, such as office visits, consultations and expertise.

A guarantor is the person financially responsible for paying the medical bills on an account. For example, if you are a parent or legal guardian, you are the guarantor for your child's healthcare bills.
As the guarantor, when you receive your next billing statement, you may notice a new guarantor number (your primary billing account number). This is a normal change, and your new guarantor number will be clearly displayed on your statement. Please use this new number when making payments or contacting our billing department.
Starting November 7, your previous Edward-Elmhurst MyChart account will permanently switch to limited access mode.
Because you will be managing your active care in MyEndeavor, you will no longer be able to send messages, schedule appointments or request prescription refills in your old account.
However, for a limited time you can still sign in to Edward-Elmhurst MyChart at any time to:
- View your historical medical records and test results.
- Pay bills for care you received before November 7, 2026. Note: payments for visits on or after November 7, 2026, must be made in MyEndeavor.
- Send billing messages regarding statements for care received before November 7, 2026.
