Residential Home Health offers skilled nursing, physical therapy, occupational therapy, speech therapy, home health aide services and medical social services. They also help patients manage more than 2,100 conditions, including cardiopulmonary disorders, diabetes, dementia and neurological diseases.

Home health care usually starts within 24 to 48 hours after hospital discharge or referral. Residential Home Health nurses and therapists are available seven days a week across the Illinois service area.

During your first visit, a Residential Home Health nurse or therapist will assess your overall health, review your medications and check home safety. You will work together to set personal goals for health and independence. Patients also receive a binder with important information for easy reference during care.

Home health care is typically covered 100% by Medicare, Medicaid and many private insurance plans, with no out-of-pocket costs for eligible beneficiaries. Eligibility requires that home health care is ordered by your physician, provided by a Medicare-certified agency, and that you are considered homebound.

Medicare covers skilled nursing and therapy visits as ordered by a physician. It does not pay for services like 24-hour care, meal delivery or homemaker services unless medically necessary.

In addition to Medicare and Medicaid, Residential Home Health accepts many private insurers, including BCBS, Aetna, Humana and United Health Care. Contact Residential for more details about insurance coverage.
