Diabetes education

Endeavor Health certified diabetes care and education specialists work with you one-on-one or in a group to create a care plan tailored to your needs. Upon referral from your doctor, physician assistant or nurse practitioner, your educator becomes part of your care team — focusing each visit on your care needs, goals, progress, and questions.

Education topics include:

Diabetes types, disease process and progression

  • Lifestyle guidance on diet, physical activity and healthy weight management
  • Medication and insulin use, including safety and dosing
  • Blood sugar monitoring
  • Reducing risks and problem solving complications, Healthy coping
  • Managing diabetes during pregnancy (gestational diabetes)
  • Insulin pump management education
  • Continuous Glucose Monitoring (CGM) management

A physician referral is required, and most insurance plans cover the cost. Free education may be available for qualifying patients.

Self-monitoring support

Your care team will walk you through blood glucose testing using your preferred tools, whether that’s a traditional glucometer, a CGM system or a paper log. 

Personalized medicine & genetic diabetes care

Through our Diabetes Consultation Program, we evaluate patients with rare or inherited forms of diabetes, including:

  • Monogenic diabetes
  • MODY (Maturity-onset diabetes of the young)
  • Neonatal diabetes

Identifying rare and inherited forms is critical to ensuring patients receive appropriate therapy. In fact, patients with certain genetic alterations may respond well to oral medications alone.

We also use pharmacogenomics to match patients with the most effective medication based on their genetic profile — offering targeted care when standard therapies fall short. Pharmacogenomics participants can also inform and improve the way diabetes is diagnosed and treated in the future.

Clinical trials and diabetes research

Endeavor Health is actively involved in diabetes research through our Clinical and Genetics Research in Diabetes program and the Diabetes Research Registry. We conduct trials in type 1 and type 2 diabetes, monogenic and atypical diabetes, obesity and cardiometabolic health.

We collaborate with leading institutions like the University of Chicago and Harvard, and many of our trials explore cutting-edge therapies and devices — like CGMs, GLP-1 medications and precision medicine tools.
