Step 1: Meet clinical agency requirements

Review the Student Vaccination Portal memo from Endeavor Health Employee Health Services. This outlines the process for validating compliance with vaccination requirements. This process also applies to Endeavor Health employees who are students.

• Students should have vaccination records ready before accessing the portal.
• The program coordinator’s name should be entered based on the rotation location; that coordinator will receive clearance.
• School email address should be used for all communications.
• Students will receive onboarding details directly from our learning management system.

Step 2: Submit onboarding documents

Complete the appropriate forms and return them to the designated coordinator for the hospital where the student or instructor will be completing their rotation. Students and instructors may have additional forms to complete to assist with on-boarding dependent on the site they are assigned.
