Friends and Family Access (also known as proxy access) allows you to securely view and help manage the health information of a child, dependent or another adult. Access levels and requirements vary based on the age of the patient.

Understanding Friends and Family Access
A proxy is a person who has been granted permission to access another patient's MyEndeavor account. Proxy access allows a parent, guardian, caregiver or other authorized individual to help manage a patient's health care online. Depending on the level of access granted, a proxy may be able to view health information, schedule appointments, send messages to the care team and perform other health management tasks on behalf of the patient.
There are two main levels of Friends and Family Access.
- Limited access: Allows you to view basic information, send messages, view letters and schedule appointments.
- Full access: Allows you to see more detailed health information, including medications, test results, visit information and notes.
Access levels may change automatically based on a patient’s age or when a patient grants or updates permissions.
To review your access level:
- Log in to MyEndeavor.
- Open the main menu, scroll to the Sharing section and select Sharing Hub.
- Select Manage friends and family access.
From the Friends and family access page, you can:
- View who has permission to see your record.
- View whose record you are authorized to view.
- Invite someone to have access to your health information (only available for patients age 18 and older who have a MyEndeavor account).
- Remove access for someone who no longer needs it.

Requesting Friends and Family Access
How you request and manage access will vary based on the patient's age.
After the birth of a baby at Endeavor Health, the birthing parent automatically receives access to their baby’s account. Access may be handled differently in situations such as adoption or surrogate births.
For assistance, contact your care team or find answers and contact options through MyEndeavor support.
If you do not have access to your child's account, parents and legal guardians can request Friends and Family Access online or in person:
Online:
- Log in to MyEndeavor.
- Open the main menu, scroll to the Sharing section and select Sharing Hub.
- Select Manage friends and family access.
- Select Request access to a minor’s record and follow the prompts to request access.
In person:
- You can request access from your care team or the front desk during your child's next visit.
When a child turns 12, a parent or legal guardian’s access automatically changes to limited access to help protect adolescent privacy.
Parents and legal guardians can continue supporting their child, but some information may be limited unless full access is granted.
Requesting full access:
To request full access, download the Adolescent Proxy Access Form. The form must be completed and signed by both the adolescent patient and their parent or legal guardian.
Submit the completed form using one of the four methods below.
- In person: Hand in the completed form at your doctor's office or any Endeavor Health care location.
- By email: Email the form to our HIM department at himservices [at] endeavorhealth.org (himservices[at]endeavorhealth[dot]org). Note: Standard email is not a secure means of communication. By emailing this form, you acknowledge and accept the risk that your health information could be intercepted or read by unauthorized third parties during transmission.

- Read the messaging disclaimer and click Next.
- Select Submit proxy form.
- Write the subject, your message, attach the proxy form and click Send.
Requesting limited access:
If you do not currently have access to an adolescent's account and only need limited access, you can request it online or in person:
Online:
- Log in to MyEndeavor.
- Open the main menu, scroll to the Sharing section and select Sharing Hub.
- Select Manage friends and family access.
- Click Request access to a minor’s record and follow the prompts to request access.
In person:
- You can request access from your care team or the front desk during your child's next visit.
If you are 18 or older, you control who can access your health information. You can invite a parent, spouse, caregiver or other trusted person to share your record directly on MyEndeavor or you can request access to another adult’s account.
Inviting someone else to have full access your account:
You can easily invite a parent, spouse, caregiver or other trusted person to view your records directly through MyEndeavor.
- Log in to MyEndeavor.
- Open the main menu, scroll to the Sharing section and select Sharing Hub.
- Select Manage friends and family access.
- Click Invite friends or family and follow the prompts to send an invitation.
Requesting full access to another adult’s account:
To request proxy access to another adult's MyEndeavor account, download the Adult Proxy Access Form. The form must be completed and signed by both the patient and the person requesting proxy access.
Submit the completed form using one of the four methods below.
- In person: Hand in the completed form at your doctor's office or any Endeavor Health care location.

Note: Some requests may require additional forms or in-person verification.
If an adult or adolescent patient lacks decisional capacity and cannot provide consent, full access may be granted to a legal guardian or a person with healthcare power of attorney.
To request access, an Adolescent Proxy Access Form or Adult Proxy Access Form must be completed.

Some types of Friends and Family Access, particularly full access, require a consent form. These forms help protect patient privacy and ensure information is shared appropriately.
Important timing for form submissions
Because we are actively transitioning to MyEndeavor, we cannot process new MyEndeavor proxy access forms until your current patient portal has migrated. Please wait to submit these forms until on or after your specific transition date:
- NorthShoreConnect patients: August 1, 2026
- Edward-Elmhurst MyChart and NCH MyChart patients: November 7, 2026
